Festoons present a very complicated eyelid/cheek problem to correct. Festoons are the overhanging hammocks of skin at the junction of the eyelid and cheek. In general, surgeons treat festoons as just one entity. However, Dr. Guy Massry has found that there are actually two different kinds of festoons:

  • Dry festoons – overhanging skin
  • Wet festoons – accumulate fluid

In Dr. Massry’s experience, each type of festoon is best corrected with a different surgical technique and the surgery should always be customized to the patient’s needs. Furthermore, Dr. Massry has found when festoons are closer to the nose and the middle part of the eye they do very well with surgery. When they are located more laterally, the skin is thicker and the results might show more scarring.

This patient had previous surgery and filler injections for to remove her festoons and did not experience good results. Her festoons were a combination of the two types – dry and wet festoons. To give her better results, Dr. Massry first removed all of the filler that was placed by another doctor. Then, during surgery he performed a direct festoon excision by making an incision over her overhanging skin at the eyelid-cheek junction, sutured it together, re-secured and tightened her eyelids and performed a lower blepharoplasty to create a smooth lid-cheek junction.

Another technique that Dr. Massry uses to perfect the results is to post-operatively inject the incision area with wound modulators. This is a medication that biologically inhibits scars from forming.

Finally, once the patient healed from surgery, Dr. Massry injected fillers in the under eye and cheek area to restore volume and create an even smoother transition at the lid-cheek junction for the most natural-looking results. This is not an easy surgery and requires a lot of expertise.

She is very pleased with her results, especially with her rejuvenated appearance.
